Based in your question and the pictures you  showed, you are a great candidate for a breast augmentation with a lift, since your skin is too saggy. You may want to consider breast augmentation sub muscular. 
Know that the size, profile, and shape of the implant is based on your desired breast size/shape, your chest wall measurements, and soft tissue quality.  This decision should be based on a detailed discussion with equal input from both you and your surgeon.

I recommend an in person evaluation by a plastic surgeon to be measured and discuss your goals. After I measure and discuss goals with my patients, we then look at pictures of former patients with similar measurements with different size and style breast implants. My patients tell me that this process is very helpful in determining what size breast implant is right for them. They also get to see what they would look like when the implants have fully dropped and fluffed and in clothes. You get to see actual results and the surgeon's experience and not a virtual idea of what can be achieved. From the picture posted, a lift as well may be needed.

Thanks for your question! I would really need to see you in person to determine what size implant I would recommend. You most certainly need a breast lift with an augmentation. However, without seeing you in person, I'm unsure as to whether or not I would recommend doing both in one procedure or a staged approach. Please be sure to consult with a qualified surgeon. Hello and thank you for your question.  The best advice you can receive is from an in-person consultation.  Based on your photographs, the best shape and contour could be achieved with a breast lift (you could place a small implant as well).  Your nipple sits below the inframammary fold and without a lift in addition to the implant, this will not have a natural shape.  This decision should be based on a detailed discussion with equal input from both you and your surgeon.  A tummy tuck can be performed at the same time.   Make sure you specifically look at before and after pictures of real patients who have had this surgery performed by your surgeon and evaluate their results.

The technique I recommend is a breast lift using The Bellesoma Method.  This will reshape your breast tissue creating upper pole fullness without implants, elevate them higher on the chest wall and more medial to increase your cleavage.  Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained.  At the same time or later, I recommend abdominoplasty with mesh reinforcement, if you are not planning future pregnancies.  The mesh creates a more hourglass figure by compressing the abdominal wall resulting in early satiety and helps maintain a reduced weight.  The combination of breast lift and tummy tuck will make your breasts appear larger without the use of implants.  However, fat transfers can be performed later if additional volume is desired.

You are an excellent candidate fir a breast lift and it can be done at the same time as the breast lift. It would take about five hours. I may be the only one to tell you this, but do the breasts in two stages placing the implant three months after the lift. Why? First of all you may not even need or want an implant with the amount breast tissue you have. Doing in two stages is safer and easier. It is very difficult to perform lift and implant in a case like yours where the nipples are that low. If you try it in one stage you have much higher incidence of complications as well as a suboptimal result. If you look at different websites, and look at breast lift with implants, most only need to be lifted maybe two inches and can be done in one stage. That’s an entirely different situation as yours. If you have it done in one stage you likely will get shortchanged on the lift part and need a revision later anyway. Your case is ideal for two stages. I’ve seen many try to do it in one with poor outcomes.  the cost difference will be negligible.

Congrats on deciding to move forward with your mommy makeover.  In regards to sizing, that is difficult advice to give you online as there is a lot that goes into sizing our patients.  We perform measurements, evaluate the skin envelope
elasticity, look at photos, look at what the patient came up with the rice
test, look at diagrams and finally use the Allergan/Mentor sizing system to
size the patient in the office.  We obviously listen and discuss exactly
what the patient wants and doesn’t want so that we can deliver the expected
results.
Your
best path to determine what size would work and how big you can be would be to
seek out a board certified plastic surgeon to discuss your options.  Make
certain the surgeon you choose has great deal of experience in breast implant
surgeries.You would also require a breast lift to achieve a nice result.  You do have plenty of breast tissue and may be a candidate for a breast lift and using your own tissues and forgoing an implant.Hope that helps.
