I am about 5'5 or 5'6 and 140 lbs I currently wear a 36 g bra. I hate how saggy/ low my breasts are.I am very interested In a breast reduction/lift but I am worried that it will only make my breasts smaller and just as saggy. What size should I go down to? And how perky do breasts get after reduction? I am thinking I want a dd because I don't want such a dramatic change. Please give pros/cons for my situation.

Thank you for the question at pictures. I find the use of goal pictures very helpful in the process of communicating with patients. In my opinion, the best way to achieve your goals would be to undergo breast reduction/lifting surgery in a first stage operation. A second stage breast augmentation would be necessary to achieve the full, somewhat “round" look at your goal picture demonstrates. In my opinion, a breast reduction/lift operation alone will not achieve the outcome that you are looking for. Also, in my opinion, given your “starting point”, I think a single stage breast augmentation/lifting operation exposes you to significant risk of complications and need for additional surgery.

Answer: Heavy drooping breasts
can be transformed into the photos your provide BUT in order to maintain that result, you would either have to consider a Rubin mastopexy (that will still sag some with time) or a reduction (possible nipple graft) with augmentation. A traditional mastopexy is doomed to failure and poor results. Discuss your expectations with your doctor and know what the revision policy as it you can almost expect it in your situation.
