Risks of declining extraction for periodontal disease and what is the best case scenario for these teeth?

53F just dx w/gum disease. Dentist recommends removing 2 teeth due to bone loss. Horizontal mobility less than 1mm in both, but one also has vertical mobility (less than 1mm). He told me about risk of infection if I don't remove. What is the best case scenario for these teeth? Is there any chance they could be retained long-term or am I just delaying the inevitable (and potentially making things worse in the meantime)? How much can good home care/treatment help?
