Your only solution is a revision after waiting for it to heal. If you’ve already had two surgeries, you need to wait a year after your second surgery. It looks like a midline platysmaplasty was done causing the thick cord. Also the platysma has been skeletonized due to lack of fat in the submental area. The two lateral swellings could be anterior digastic muscle or submandibular gland. Your revision should not be more of the same technique. Your third surgery needs to be the last one so you should choose your surgeon carefully. The skin needs to be elevated leaving as much fat under the skin as possible. Then the platysma needs to be elevated but no muscle excised. Any submental subplatsymal fat should be leveled but not overresected and finally the anterior digastric muscle and submandibular glands addressed and partially resected…all done under the platysma to keep the result smooth. Lastly you might consider a deep plane lift incorporating a platysmal hammock to pull in the lateral submandibular area. Best of luck in your search.
