Question

51 years old, 5 feet two inches - 126lbs - had filler a few years ago on sides of mouth (i wonder if aome has migrated worsening jowls?) - I so not want a facelift. Am i a good candidate for ultherapy? If not could fillers ir sculptra provide a bit of a lift? Not looking for perfection. Also concerned about under my neck.

It’s hard to grade mild vs. moderate without seeing you in person, but what you’re describing sounds very common in this area and age range.

Filler migration is a common concern, but true migration significant enough to create jowls is actually not very typical, especially years later. It’s more often a combination of natural volume loss and skin laxity.

Ultherapy can be a good option if you’re looking for some tightening without surgery, particularly along the jawline and under the chin. Just keep expectations realistic; it can improve things, but it won’t replicate a facelift.

Fillers or biostimulatory treatments can help restore support and subtly improve contour, so in many cases it’s not one or the other. A layered approach, addressing support and then tightening, often gives the most balanced result.

An in-person evaluation is the best way to determine what’s contributing most and how much improvement you can realistically expect.

Since you are clear about avoiding a surgical facelift and are not looking for absolute perfection, we must manage expectations correctly. No non-surgical device will provide a surgical result. However, a strategic combination approach can offer significant improvement:
Energy-Based Devices (Ultherapy or RF Microneedling): Ultherapy can be beneficial for deep tissue contraction and stimulating some collagen, but relying on it alone will likely fall short for moderate jowls.
Biostimulators (Sculptra or Radiesse): Since you mentioned Sculptra, this is an excellent choice. It acts globally to stimulate your own collagen production, improving skin quality, restoring lost volume in the midface, and providing a subtle "lifting" effect over time.
