My 2nd bicuspid (#13) is helpless and needs to be extracted, so tooth implant vs orthodontics? (Photo)

Hi doctors! My 1st bicuspid (#12) was extracted several years ago. A periodontist told me that he would extract My 2nd bicuspid (#13) and then do the bone replacement graft on the same day. I'm wondering is it possible to close the gap by pulling my 3 molars forward with orthodontics? Also I'm thinking can I have the extraction first and then do the tooth implant or wear brace years later when I have enough money to afford it? What are the consequences if I do so? Thanks a lot!

Hi  The best treatment would be to have the extraction with bone graft then come back within one year and place the dental implant.  The braces to pull the 3 molars forward to close the space would throw your bite off and not be ideal.  The longer you wait to do the implant then the less likely the bone will be ideal for an implant.  Other options for tooth replacement include a bridge or partial.I hope this helps.  Good luck.

The best route in my opinion would be to extract the tooth and do bone graft on the same appointment.  4-6 months later place an implant.  I would not suggest to move all the back molars forward due to distance of the space and future bite alignment!

That is a very good question. Orthodontics is not a good option for a couple of reasons. First of all, moving molars orthodontically is very hard to accomplish. Think of it as a tug of war between your front incisors and your back molars. The molars are going to stay pretty much where they are. It would also significantly increase your risk of root resorption and TMJ. The appropriate thing to do is bone grafting and a dental implant. Some dentists have also gotten really good at placing the dental implant at the same time too - an immediate dental implant. This limits the needs for additional surgery and can help to stabilize and maintain the the integrity of the bone if done correctly. Sincerely,Sarasota Dentistry - Hank D.
