Dermal fillers and scar tissue

My doctor effectively dredged out the whole jawline and chin when injecting hyauloronic acid Volux fillers. He even commented that he was effectively ripping the skin away as he went along. Is this how it’s always done each time you come back to top off the fillers after 12ish months? It seems fairly invasive and aggressive (bruising / swelling etc) Because I would imagine a ton of scar tissue eventually forming from all the trauma and post-procedure inflammation.
