From the sound of it, subcision, a procedure ordinarily reserved for releasing tethered down scars, is being performed along with the injection of the filler. I have been performing subcisions for elevating and smoothing all kinds of scars since 1995 and have been injecting fillers for aesthetic reasons for well over three decades. In general, subcision, which sounds like what is being performed here, is not needed unless there are circumstances in this case that have not been mentioned. And, yes, very aggressive subcision does ("ripping the skin away") could conceivably engender an overabundant production of new collagen and elastic fiber synthesis that could conceivably evoke the kind of situation that you express concern about. When we are discussing jawline filler, the product needs to be placed right on top of the bone or as close to the bone as possible. There are different techniques people use to do filler, either direct injection with a needle or a cannula technique which threads the product in. In response to you question about scarring, there is some scar tissue that develops after any sort of tissue trauma but in the grand scheme of things, filler placement does not produce an abundance of scar tissue.
