You would benefit from an eccentric lip, not a bullhorn lip lift. This does 4 things: shortens the distance between your nose and upper lip, increases the mucosa of the upper lip, increases the incisor show and pulls up the corners. This takes 1 hour to perform under local anesthesia and costs $4,000.

It appears that both jaws are smaller and will need to be moved forward? That needs to be evaluated based on clinical and x-ray evaluation. Lip lift with the scar hidden in nose/lip crease would make upper lip more prominent and larger; correction of lower lip with filler augmentation or surgery may be needed for more symmetry. In person consultation with a craniofacial or maxillofacial surgeon is recommended. Good luck.

I've been doing lip lifts longer than just about anybody practicing right now. There are several variations, but you should be able to get a good result from what I can see. It is done in the office in about 30 minutes.

I’m going to be very honest here—this is where I’d slow things down.

At 20:

Your facial structure is still relatively youthful
Small proportional concerns often look more dramatic to you than to others
Lip lift considerations:
Permanent procedure
Leaves a scar under the nose
Can feminize the upper lip if overdone
My usual approach in your case:
Start with no surgery
If anything:
very conservative filler (if needed)
or simply observation
Bottom line:

I would not rush into a lip lift at 20 unless there is a very clear anatomical indication.
