Given the time since my last course what cumulative dose do I need this time around (second round)?

I'm a 45 year old 75 KG male who was on Accutane last year. Started in March 2020 and got off in July 2020 per my dermatologists. Dose was 40mg month 1&2 then 60mg month 3&4. Only bad side effect was terrible dry eye. After checking the math I don't think I hit the Target Dose of 120mg. Fast forward 7 months later and my skin is breaking out again. Dermatologist wants me to go on round 2. I'm all for it. Given the time since my last course what cumulative dose do I need this time around?

Accutane is a go-to medication for patients with recalcitrant cystic acne. It can reduce pore size and also reduce sebum production in your skin, making it less oily; and in turn, dryer. Accutane can be used to control acne, but patients receiving the medication should be carefully monitored. In terms of skincare, always wear an appropriate sunscreen and reduce direct sun exposure. In my patients, I recommend a good ceramide moisturizer such as Elevase whenever they are on Accutane. This will keep the skin hydrated and reduce the oiliness of the skin that is associated with oil-based moisturizers.

Thank you for your question. Isotretinoin therapy is weight based treatment, and the proper cumulative dosing is 120-150 mg/kg. Also, most brands of isotretinoin is required to be taken with a high calorie and high fat meal to maximize absorption. I highly recommend you consult with your dermatologist regarding further questions and concerns about your treatment. Take care and good luck.
