       I think that weight loss is not an unreasonable suggestion prior to consideration of liposuction.  In most outpatient settings, the surgeon is limited to 5 liters of liposuction.  You really want the results to be as good as possible.  Liposuction results are highly surgeon dependent.  Find a plastic surgeon with ELITE credentials who performs hundreds of liposuction procedures each year.  Then look at the plastic surgeon's website before and after photo galleries to get a sense of who can deliver the results.

A bmi of 38 is very high and liposuction it not an answer for weight loss. Best to get down to your goal weight and a reasonable BMI to be re-evaluated.
