The dentist installed an implants however the tooth is higher and larger then the other front tooth. The left front tooth has a crown as half of it broke off. Shouldn't the dentist have fixed the gum prior to the implant (bone grafting?) and now is it too late to fix? Also the shape is off and both teeth are discolored. Any advice will be gratefully appreciated. I spent lots of money to not be satisfied with my smile. I just don't know how to go about it- its been a few months now since the "fix".

This is a common problem that I have seen over the years when treatment is not planned out properly. Gum reaction is forecast PRIOR to implant placement and crown cementation. Now, depending on the severity of the gum recession and possible bone loss, it is possible that the whole treatment might have to be redone in order to achieve a satisfactory result. If you are lucky, a simple gum graft might help. But it is not clear what can be done based on your photo. If you could submit a better photo we might be able to see better.

Answer: Implant first thing i want to tell u is , its a artificial structure and not natural one so never expect excellent result like natural from artificial things. in usa and canada people go for implants frequently ,but never consider as a last treatment option. now come to point. in yr case its because of bone loss in implant region [ prior to implant placement] , thats why implant placed bit up. u can achieve ideal result by implant placement followed by gum surgery for proper bone creation. that is not possible with bone graft . in yr kind of cases it is possible to create ideal bone by taking bone piece from other body part and placing at implant side. in this procedure also results are not always as per expectation , so u have to consider this thing.now u have two options 1. just replace crown , and tell yr dentist to add pink gum shade in gingival area[camouflage] or.2. remove implant , follow treatment as i described above.

Implants in the cosmetic zone can be quite complicated. I would suggest you find a specialist in your area and have a consultation with them to see what can be done to correct the cosmetic concerns. Its not very clear from your picture but it seems like the gums receded after the crown was placed. There are various reasons for this from extra cement trapped under the gum, to gum recession caused by bone recession. Most of the time and most people don't show the gum that high when they smile so the problem is hidden but in some with a high smile this is a very serious issue. The best is to go back to your dentist and see when he suggests, if he can't do anything consult with a dental surgeon that has experience with treatment front teeth with dental implants.
