This is a question that if you ask 10 different rhinoplasty surgeons, you may get 10 different answers. Some surgeons recommend saline sprays immediately after surgery, some do not. Saline irrigations I reserve for my patients that have had sinus surgery done at the same time as rhinoplasty. In those cases, I tell them not to start irrigations until at least one week after surgery, and I instruct them on the proper technique so as to avoid traumatizing sensitive areas; therefore, I don't believe there are absolute do's and dont's on this topic. You may want to double check with your surgeon as to exactly what they would like you to do.
I have my patients use saline sprays the day after rhinoplasty or nasal surgery. I have them start with small amounts in the morning. I advise against the use of nasal irrigation using Neil Med or large bottles with high volume as this is too much pressure and can disrupt the healing of incisions.
I instruct my patients to use nasal saline spray a day after their septoplasty or rhinoplasty surgery. The spray is used 5-6 times per day until all crusting within the nasal cavity is resolved, generally about 10-14 days.
