33, female, prolonged swelling/sagging post Taylor Liberator subcision, causes & options? (Photos)

Question

I am a 33 year old female. I had full field Taylor Liberator subcision performed 2 years ago to try to improve acne scarring on my cheeks. I have been left with a swollen face where volume in my cheeks appears to have dropped. I have waited this long to see if it was swelling and my slim and contoured face would return but as you can see it hasn't. I feel I have aged quite significantly. What could be the cause of this and what are my options?

I have been performing subcisions, both individual and Field Subcision, since 1995 with gratifying results for elevating and smoothing indented scars and widespread acne-related atrophy of the cheeks, temples and chin, respectively. These treatments have a proven, several decades' long track record for both safety and efficacy in all skin types. I do not, however, perform the Taylor Liberator procedure, but have treated quite a number of people who have come to me with complications of this approach, which is a very aggressive form of subcision that appears to engender in the cases I have treated significant tissue fibrosis, which may account for the swelling described in the question. Fortunately, I have been able to achieve significant improvement using an algorithm of intralesional injections of varying combinations, concentrations and amounts of anti-inflammatory agents to shrink the fibrosis. You would be wise to consult with an experienced, board certified cosmetic dermatologist and best of luck.

It’s difficult to make an assessment based on the pictures you’ve included. All people have significant facial asymmetry, so we ideally should see full face images. We generally make facial assessments without facial expression.
I suggest you follow up with your provider and discuss your concerns with him or her.
