The buccal space is a loose areolar space which means that it is spread over a wide area and with loose attachments. It is very difficult to inject far into this space. Start with masseter botox to slim the outermost part of your jaw. It's easier, reversible, and may give you the result you're looking for. Would not recommend a facelift at this point,

Buccal fat removal will only thin your cheeks and could make the tissue laxity even more severe. Looking at your photos a facelift would be the procedure I'd recommend. If you reposition the soft tissues I doubt you'd need a buccal lipectomy.

I don't believe that you are a good candidate for buccal fat pad reduction. The fullness of your lower face is due to the angle of your jaws and massater muscle. I would recommend facial re-shaping with a reduction of the angles of the mandible and submalar cheek implants to add fullness to the...
