In your case, implant size shouldn’t be chosen based on a number alone it really depends on your chest anatomy, and for a natural look I usually recommend a conservative, proportional approach. If you are open to it, starting hormone replacement therapy can help stimulate some breast development and improve the soft tissue, which often leads to a more natural result with implants, but it’s not mandatory. Regarding implant placement, the submuscular plane is safe and has been used for a long time, especially in very thin patients, but in transfeminine patients the pectoral muscle tends to be stronger, which can lead to more noticeable implant movement or displacement over time. Because of that, when anatomy allows&mdash;particularly if there is some existing tissue. I often prefer a subfascial (prepectoral) approach, as it tends to provide a more natural look and movement.
