Fractional lasers such as Fraxel, erbium, and CO2 laser can be combined with radio frequency and also fractional RF to improve textual issues and microtopography. Subcision and TCA cross are also used for deep ice pick scars and narrow valley scars in order to improve the micro texture. This keloid will be quite difficult to remove without surgery. I would recommend surgical excision with steroid injection (kenalog) right into the site. This will treat any remaking scar tissue and help to ensure that the keloid does not recur after treatment.

With severe recurrent keloids I recommend excision timed with a series of radiotherapy from a radiologist MD/Radiation MD.
