Based on your photos, you appear to have a dorsal hump and a hanging columella. Generally, these issues are addressed with rhinoplasty surgery. Threads were never designed for the nose, and they do not affect anything more than 6 months. The nose is a complex structure of cartilages of various sizes, thickness, and shapes that are interconnected either firmly or loosely with ligaments to themselves of to bone. A rhinoplasty does not mean a cookie cutter result, and that may be driving your partner away from the idea. Using digital simulation technology, you and your surgeon can plot out how you want your result to look and maximize the potential for a satisfactory result.

Unfortunately, no photos were available for me to view. However, in general, I have successfully used HA fillers for treating hanging columellas. First a bolus of a more robust filler is placed at the base of the nose where it joins the face. This typically works well for raising the base and improving the profile view while diminishing the mucosal show in profile. Next, a more cohesive filler is placed in microdroplet injections along the alae further decreasing the amount of mucosal show. As injecting filler in the nasal region should not be left to the hands of novice injectors, make sure to consult with an experienced, board certified cosmetic dermatologist with experience in nasal filler injections. I can virtually assure you that a thread lift will only be a temporary fix. Try it if you like but it will be a waste

Hey full set of facial photographs from all angles are going to be required to make a determination about how best to proceed with a full rhinoplasty versus a columella-plasty. It is possible to perform just a columella-plasty under local anesthesia,but that will be rather painful to get that entire area numb in order to proceed with the surgery. The columella-plasty usually involves trimming extra skin and cartilage in that area.
