Hair loss and extreme shedding: all my hair have white bulbs. (Photo)

Hey I want to ask about hair loss I'm facing my hair are shedding like hell its like a whole bunch come in my hand while I comb shampoo or anything I don't get if its telogen effluvium or anagen cuz I'm loosing my baby hair along with it plus I've a fungal infection on my scalp for like 3 years dandruff comes and goes I took ketoconazole topical for it but then I got it back ive very itchy and flaky scalp,all my hair have white bulb thing but they feel dead ..kindly tell me the best treatment???
