Question

Your last answer to my question explained everything, and you described the left bone breaking just the same way my surgeon did. You asked me to post better pictures so I'll do that. I really appreciate your opinion. Will spreader grafts make my nose wider? Is having the bridge I once had with good contour and even nasal bones back achievable or should I expect less? By lateral and oblique views did you mean x-rays? I've attached better quality pictures.

Thanks for the images, they are the ones I meant.I think you can get even a better profile and dorsum than you had before (I mean the profile view).Your supratip is currently thick and broad, but your middle vault is pinched, a contradiction? not that much:-the broadness of the middle vault is due to fibrosis: solution is debridement, meticulous-the collapse of the middle vault is due to the open roof issue: solution is spreader grafts, but WELL sized and never thick ones, otherwise you'd look like a flad and square middle vault, it is a matter of good aesthetic judgement and accurate calibration of the gesture, like in every rhinoplasty maneuverThese images are excellent and clear, they confirm all I told you; additionally:-the assymmetries you notice now at the nostrils were pre-existant and your surgeon is not to blame; they have no solution, it is natural having uneven nostril sizes, we all have such difference-your nose looks more pinched than I imagine, you really need batten grafts and correction of this unsighty effectYes, your case is sadly a mess and the difficulty high.Once again feel free to contact me through my profile form if you wish further comments.

Spreader grafts will tend to widen the middle of the nose a little. This varies based on thickness and placement of the grafts. In your case, a graft just on the left should improve symmetry. There are some other issues, like smoothing the bridge and lowering alar flare (arched nostrils) that could also be addressed. Thanks for your questions. Spreader grafts can be placed on either one side that is concave, or both sides that have the hourglass deformity. Depending upon the thickness of the graft, they can make the nose slightly wider in that area. For more information about spreader grafts and many examples, please see the link and the video below
