I had my reduction/lift 3 months ago with some delayed healing with the T-junction on one side but other than that was fine. The incisions have been scarred over for a month but yesterday I notices a hole in the vertical incision on the same side that had issues earlier. It's about 2 centimeters in diameter and looks like the scar tissue pulled apart at that point. It looks almost like a puncture wound. I can't contact my surgeon until the end of the week and just wondered what I should do?

Yes it is. You need to excise skin from the lower pole of the breast which will pull the areolar complex down. You can also redo any scars that bother you and also make the breast slightly smaller if you desire.
It is unlikely but not impossible that pressure cause a problem. It is more likely associated with your musculoskeletal system as opposed to the breast itself. Discuss this matter with your surgeon and chiropractor. Best wishes.
