Are temple implants my best option to address this hollowing/volume loss? See the attached pictures. My hollowing is significant, in my opinion. I thought it might require a lot of filler material — whether it’s HA, Sculptra, fat, etc. — to address this level of hollowing. And I worry about the risks that come with fillers, even though they’re rare, because it’s something you have to do over and over again (not a one-time fix).

Thank you for your question and photos. It appears you would be an excellent candidate for injectable dermal filler or a collagen stimulator like Sculptra in this area. Both of these treatments provide natural results when administered by an experienced, board-certified injector, and the improvements can last up to two years. For even longer results, you might consider fat transfer, a procedure that removes fat from one area of the body like the abdomen and places it into the temples via injection. The results of fat transfer can be permanent for some patients. I encourage you to schedule an in-person assessment with an experienced, board-certified facial plastic surgeon for a comprehensive examination and customized treatment plan based on your precise needs and goals.

In experienced hands, fillers can work well for augmenting and smoothing sunken temples. Although there are risks with fillers, in experienced hands, fillers have proven safe and effective. temples can be improved with a combination of fillers and sculptra. long term bellafill works great too (see link).

most of my younger clients use fillers to give them shaping especially on the cheeks, jawline, and chin which make the face look more slim and tight; and threads such as PDO or Instalift can build collagen and support the facial shaping and skin looseness over time, slowing down the aging process and given the face a more “snatched” look; and morpheus deep microneedling RF can tighten crepe skin especially on the neck and jawline and is great for long term tightening of the full face, neck and chest; its often used to give the face a slimmer more sculpted look because it tightens the skin and gives lift.  an early face lift can be performed as well even in younger clients if they want a different facial shape, maxillofacial surgery with bone breaking or shaving can also be done.

For facial slimming a combination of treatments are always needed.  Options include:
1. Buccal fat pad removal to reduce fat on the lower face
2. Factite or Renuvion/J Plasma to tighten the lower face, jawline, and chin to reduce fat and give the jawline and neck tightening and shaping.

In my experience, temple implants don't work very well because the skin there is very thin and you can usually see the edge of the implants.  Injectable fillers can blend much better.  Temples are commonly treated with fillers.  There are risks with any procedure, but an experienced injector should be able to safely help with your issue.

Thanks for the photos!  You do have significant hollowing of your temples. However, I would strongly recommend filling rather than surgery since this will give you the most natural results. Yes it is true that filler does not last forever, but once you are satisfactorily filled, you will not have to come in frequently for maintenance. Since our faces continue to change as we get older, use of filler allows us to adjust to these aging changes whereas a surgical implant remains the same and can become very obvious in its placement over time. I would suggest you consult with a physician who is very familiar with the anatomy in the temples to further assess your temples and help you to decide on your best options. Temporal Implants questionThis day and age you absolutely do not need any kind of implants just about anywhere on the human body. That grafting is so effective that it produces natural and permanent results as long as the procedure is done by a plastic surgeon with lots of experience in fat crafting techniques.
