Based on the information you have provided, it seems that you exhibit a fairly high BMI unless you are super muscular.
At this age, I would advise you to consider loosing weight to reach a BMI of about 25 and then reassess the situation of the gap between your breasts as well as the 44DD size you have now.
If none of this helps and you managed to stabilize your weight for a BMI of 25 or less, then considering a breast reduction may be a solution.

While it would require a physical examination to fully answer the question, your issue does have a resolution. Before offering any surgery, I would need to know if your height and weight are stable and unchanging. Weight loss is always an excellent suggestion, but unlikely to solve your problem. It sounds like you quite simply need breast reduction surgery for both comfort and cosmetic reasons. If your height and weight are stable, and the breast is a significant burden, then age becomes less important by about 16. Often breast reduction surgery results in patients more easily pursuing weight loss and excercise in the future. Lastly, most surgical techniques do not eliminate your ability to breast feed in the future. Good Luck
