Intralesional injections of anti-inflammatory agents is the treatment of first choice for keloid scars in order to flatten them, lighten them in color and to treat itch and tenderness. Surgical removal, especially in an area subject to a great deal of movement (such as the knee) poses a high risk for recurrence and perhaps even a bigger scar. I have been successfully treating keloids for several decades and jumping into surgery, while it may seem like the logical first approach, typically is not.
