I have PCOS and can't get my hormonal acne under control, it's affecting every aspect of my life and I need help. Advice?

I've been dealing with hormonal acne on my face & back for years and after blood work I was diagnosed with PCOS. I've been to many dermatologists but all they do is prescribe tons of antibiotics and clindamycin gel. OBGYN put me on birth control pills but I had terrible side affects and had to stop. Tried different bc pills too, nothing helped. I go to Mexico to buy Retina which works well but I still break out. Tried spironolactone, had a heavy period EVERY single day. Please help, thank you!

I have a lot of patients who have cystic acne.  You may be one of those patients who need a combination of Accutane and an oral contraceptive pill to have your acne under control. You need a complete panel of blood work including hormones such as testosterone and thyroid panel.  I wouldn't use anymore antibiotics since that hasn't worked.  You need to consult with a specialist in acne and acne scars and you may need to see an endocrinologist as well.  Best, Dr. Green

Thank you for your question. I'm sorry to hear about your struggles with acne and PCOS. From a dermatology standpoint, other acne treatment options include Accutane, different topicals, light therapy, and spironolactone. However, if PCOS is uncontrolled, then acne will continue to return despite treatment, so I highly recommend you consult with your OBGYN regarding alternative treatment options to help with PCOS.
