You may have some atrophy of the tissue following steroid injection. However, since it is at the end of the scar, if may also just be an indentation resulting from the edge of the scar. I would try some massaging and giving it some more time.

If the indentation persists, you may benefit from fat transfer to help push the indentation back out. You may require multiple sessions of fat transfer to correct any contour deficits. You don't want to over do the fat transfer and end up with an area of fullness.
