I‘ve had breast augmentation 8 months ago and think it looks quite unnatural. 170cc Motiva Ergomix dropshape. Could a fat transfer help or would just placing implant under the muscle help? Or are the result good enough?

In your photos the outline of the implant is visible at the upper and inner edge, which tells me there is not enough tissue covering it, and it looks placed over the muscle. In a chest as slim as yours that placement almost always shows. Fat transfer can soften the edge a little but you have very little donor fat and a good part of what is injected is absorbed, so it would be an expensive partial fix. There is a second point I want you to notice, your nipples sit quite far to the outside of the chest. If a small implant is placed close to the midline to create cleavage, the nipples end up on the outer edge of the mound and look odd, so a small implant is actually working against you here. The better plan is to move to a dual plane pocket under the muscle with an implant that has a wider base, so it covers more of the chest wall and the nipple sits in the centre of it. Wider base does not have to mean bigger, a low profile implant with a broad base gives coverage without much extra volume. I would also choose a more stable implant than the Motiva Ergonomix, which is very soft and can shift and rotate inside the pocket, especially in a thin patient with little tissue to hold it. Wait until a year from your first surgery before revising, the tissues need to settle first.
