Over three years post blepharoplasty - will CoolPeel tighten the dog ears and divots? (Photos)

Had botched upper blepharoplasty 39 months ago. Have horriblel lateral lid divot and dog ear. My eyes are different shapes now as is but this excess skin and flap make it impossible to disguise with makeup. Will coolpeel of eye area help fix this or an office dog ear surgery to help?

After 39 months, the tissue from the original blepharoplasty is well healed, so the key question is what is actually creating the lateral “dog ear” and divot. If the issue is primarily skin laxity or a superficial scar/texture irregularity, a fractional CO₂ treatment such as CoolPeel may provide some tightening and improve the surface texture. Fractional CO₂ resurfacing has been shown to improve periorbital laxity, wrinkles, and some types of scarring.

However, if there is a true dog ear, localized excess skin, tissue tethering, or a structural indentation from the previous surgery, I would not expect CoolPeel alone to completely correct it. Laser can remodel the skin, but it cannot remove a discrete flap of excess tissue or release a deeper adhesion. In that situation, a small surgical scar revision or dog-ear excision may provide a much more predictable correction.

Because this is around the eyelid, I would be particularly conservative. CO₂ laser treatments in the periocular area need appropriate settings and experience because excessive treatment can cause pigment changes, scarring, or eyelid retraction.

So, in simple terms: CoolPeel may help soften and tighten the area, but it is unlikely to completely fix a true dog ear or significant divot. If the problem is structural, a minor surgical revision would generally make more sense than trying to laser the tissue away. The photos can help suggest what is going on, but an in-person examination is important before deciding between laser and revision surgery.
