At least from the description, this issue here sounds like an adverse under-eye filler complication that has recently been called "puffy pillows," which essentially are soft protruding pockets of lymphedema (lymphatic obstruction).located in the delicate infraorbital region. It has been associated with other fillers, as well. And even the use of dissolvable HA fillers may result in this problem, especially if an inappropriate choice of filler is chosen for use n the under-eyes and/or injected too superficially. I have been able to achieve gratifying flattening of these puffy sacs in patients who have come to me having been treated with HA fillers, as well as permanent fillers, using an algorithm of varying anti-inflammatory agents in varying combinations, concentrations and amounts.
