There is so much information surrounding these two treatment modalities, yet I'm confused as to what may be most beneficial. For a raised, hypertrophic scar (non-keloid) what does the best job of flattening w/o causing side effects? Thanks so much!
Answer: Kenalog/5-FU Tried & True Treatments For Hypertrophic Scars Intralesional Kenalog, either alone or in combination with 5-FU, a relatively inexpensive treatment approach for this kind of problem, has proven itself a tried and true treatment modality for hypertrophic scars, in particular, as well as for keloids, for decades. I have personally used this simple approach for over thirty years myself. In comparison, relatively expensive, and heavily marketed and advertised light and energy-based therapies for these same purposes are comparative newcomers on the scene, and, as for just about all of their purported virtues for many indications, have yet to be fully proven through rigorous science. Make sure that you consult a board certified aesthetic physician with experience and expertise in treating hypertrophic and keloid scars. Best of luck.

Hypertrophic scars are those that are raised above the skin surface. Treatments include fractional ablative or non-ablative resurfacing and injections such as steroids or 5-FU. Often times a combination approach works best. IPL will help if there is color abnormality such as darkness or redness. Please consult with a doctor for specific recommendations. Good luck!

There are studies that show benefit of each of the modalities - different lases and injectables but I don't know of any that compare different combinations of therapy. If a scar is raised and not red I use a fractional erbium laser (Sciton Profractional) with or without steroids/5FU - depends on the thickness etc. For red scars I use a vascular laser - I like IPL/BBL but other vascular lasers are excellent as well. Combinations of these therapies are also used depending on the scar and previous treatments. Scar revision is also used first if the scars are terrible and lasers/injectables are used in the healing phase. Best to see a doc who has experience and a variety of devices/lasers
