I think that you will need a combination of treatment to achieve your stated goals. I would recommend a chin implant or genioplasty (depending on the amount of forward projection needed) and submental and jawline liposuction. Once healed, I would re-evaluate the cervical mental angle to determine if further surgery is required. In some case, no further treatment is needed. In other cases, a platysmaplasty may be needed.
