Renuva turned area hard, brown and painful when pressure is applied. What could have caused this? 3 years post op (photos)

I've had an injury by cool sculpting from a cosmetologist who wrongfully deemed me a candidate. I was/am skinny. It is an empty dark cavity where my adipose tissue needs to be restored. I need to have more Renuva, but I've had 12cc. A small portion of Renuva after 3 years has turned hard, brown and when I apply pressure it hurts a lot. My doctor does not know why. Did the doctor not inject it properly? Next to it the rest of Renuva did not turn brown and it developed well.
