Would buccal fat removal help my lower face? (photos)

Question

I'm 54 and have always had a bottom heavy face. I had a mid-face lift and cheek implants years ago but am still bothered by the boxiness in my lower face. I'm a very fast metabolizer, so injectables don't give me much "bang for the buck" (fillers last just over a week and botox lasts 2-3 weeks).Would buccal fat removal help? Or do I need a full facelift? Thanks so much for your opinions.

I would recommend a facelift at this time. This will elevate the soft tissues of the midface reduce the lower jowling. Simultaneous fat grafting may be beneficial.

Looking at your photos, I believe a combination of a full facelift and fat grafting to the upper cheeks and along the nasolabial fold with full face Halo would make the more dramatic change and give you a more youthful appearance.

You have a narrow lower jaw (and shallow midface-implants helped) so the soft tissue will drape over the jawline. A face-lift is the best approach to recontour the area
