It is hard to tell, but it looks like you had a vertical lift, which is a good thing. Howerver, your nipples are too high on your breasts. This is a common problem in cosmetic breast surgery unfortunately. The only correction is to have more surgery to further tighten the lower pole of your breasts so your nipples won't appear to ride so high. Even this does not totally correct the problem, but makes it better. Redo surgery would not create additional scars. The same vertical scar would be used. I've attached a link with some photos of my own patients.
There is no way to directly lower the nipple position without a scar above the nipple which would leave a visible scar in garments. However, the distance from the nipple to the breast fold is quite long and could be shortened with a lift. The removal of skin and tissue from the lower pole of the breast will once again place the nipple in the central breast over the most projecting part of the implant. This can be done with your current implants. Seek out a board certified plastic surgeon to consult.
I agree with the other responders that additional surgery will be needed to remove more tissue under the breasts to get the implants positioned higher. The other option may be to wait out further descent of the nipples with gravity, but this will need adequate support of the lower pole so they don't bottom out further. Moving your implants up and removing some of the tissue from the bottom of your breasts will help center your nipples more and may help keep them from popping out! But just lowering nipples always leaves a visible scar that can pop out of clothing as well...
A lot of successful plastic surgery can be due to illusion. Elevation of the implant position, removal of some of the tissue in the lower central portion of your breasts but leaving the nipple position where it is, will create the illusion of moving your nipples lower and create a more desireable visual relationship between the nipples and your breast mound. You cannot move the nipples lower per se without a very noticable scar above your new nipple position.
You asked can the nipples be lowered and centered. However, in the photo, it looks like the distance from the nipple to the fold is too long, and perhaps by shortening that distance your breasts would look better. An exam in person would be most helpful to figure this out.
