Under my Eyes Looks Terrible when I Smile. Why? (photo)

Botox and Dysport work by relaxing the muscles that create lines around the eyes. This reduces the wrinkling. In older patients, we need to be careful treating lines in the lower eyelids. The fat that creates eye "bags" starts to pooch out in the 30's to 40's. Relaxing the muscle around the eye (orbicularis muscle) can make those bags look worse. Whats good about these treatments is that if you don't like the results, it will wear off in a few months. The bad thing is that patients usually like the results and it....wears off in a few months.
 

Eye wrinkles are a common concern, and it is something we can definitely improve!  You would do very well with a neuromodulator (eg Botox or Dysport) - Neuromodulators prevent your obicularis muscle from contracting when you squint or smile.  Neuromodulators help with crows feet, as well as the "jelly roll" that forms under the eye when you smile.  (Personally, I like the jelly roll, but this is a personal/cultural preference).  I would also recommend protecting your eyelid skin from the sun and using a good eye cream, preferably one with retinol if you can tolerate retinol).

Botox, Dysport, or Xeomin can inhibit the movement at your areas of concern, producing a slightly more bright-eyed look.  If after that you still feel that you need a little something extra, consider adding back in some volume to the area below the eyes with a filler such as Restylane.  Seek treatment from board certified plastic surgeon or highly skilled and experienced injector and start conservatively, as a little bit of both products goes a long way in the area.

With repeated muscular contractions, the overlying skin creases. This is what’s happening with you. Every time you smile, you squint a bit, and that forms all of those wrinkles. If the skin didn’t wrinkle, your smile would probably not look as genuine. For that reason, it’s totally natural! If you want to minimize the wrinkles you see, you can consider Botox for the outer edges of your eyes, however it’s important to understand that not every wrinkle will disappear when you smile.

You may not need Restylane under the eyes, the Botox will at least take care of the wrinkles and make the eyes more open

Wrinkles that get worse with the movements of the muscles of facial expression are known as dynamic wrinkles. In the early stages of wrinkle development, little, as in this case, is seen at rest. However, sun damage and genetics can lead to a situation that when emoting wrinkles and crinkles appear. Later on, the continued folding of these muscles can "etch" the skin and engrave permanent marks that are present even at rest. 

Neuromodulators, such as Botox, Dysport, and Xeomin, are excellent choices for damping the movement of these muscles sufficiently that the crinkling is far less prominent upon motion. A secondary benefit of such treatment, especially when performed early on, is that the fewer times a muscle of facial expression contracts and the less forcefully it does so, the less likely it will be to etch a permanent wrinkle mark at rest.

     This is just muscle contracting causing skin wrinkling and is normal.  Botox can reduce the muscle action and help the wrinkling.  I would not use filler in your case.  Kenneth Hughes, MD Los Angeles, CA

Having some Botox or Dysport injected in the crow's feet area around the eyes and Restylane to fill the tear trough would yield a great result that I think you would be very happy with. The Botox/Dysport generally lasts 3 months on average. Restylane lasts an average of 6-7 months, but generally longer in that area. I would recommend an in-person consult with a board certified dermatologist, plastic surgeon, or facial plastic surgeon in your area. They are all great choices. Based on your photos, I'm guessing the 'bags' that you dislike represent bunching orbicularis muscle, rather than excess lower eyelid skin. You also have very mild volume loss in the mid-facial area. A combination of Restylane or Juvederm, plus a small amount of Botox in the lateral and central orbicularis muscle around your eye might soften this look.
