Hair transplant surgery is a clean surgery (not a sterile surgery), so the risk of tetanus is virtually non-existent. That being said, it is a good idea to be up to date on vaccinations from a general health standpoint, but really has nothing to do with the procedure itself.

There is no procedure to change the hair colour. You could colour your hair so that all the hair would be uniform.
If your surgeon does a proper closure and you heal properly, you should have a flat area. If you have any problems and concerns you should contact your doctor for an evaluation.
