Your breasts are asymmetrical in volume, shape, position and very low on the chest wall. The technique I recommend is a lift using The Bellesoma Method. This will reshape your breast tissue creating upper pole fullness without implants, elevate them higher on the chest wall and more medial to increase your cleavage. The weight of the breast is transferred to the underlying pectoralis major muscle resulting in pain relief and long term stability. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained. Later, fat transfers can be performed if additional volume is desired. Dear khamhon,determining whether you need simply a breast augmentation versus a breast lift can be somewhat complicated. It depends on a number of factors including skin laxity and current nipple position. Generally speaking, if the nipples are lower than the inframammary line the patient will most likely need a breast lift. If the nipples are at or above the inframammary line we can generally get by with just a breast augmentation depending on the size of the implant the patient wants. I often still using implant when I perform a breast lift because it provides more upper breast fullness after the lift. Just based on your photos, you would do well with a lift and augmentation. There is no need to reduce your breasts and place a larger implant.

Usually the downsizing leaves the patient with extra skin that does not fully contract. This leads to an appearance of breast tissue hanging off the implant so usually the surgeon will recommend a lift to remove the skin excess.
I do think a minimal lift is all that is needed to correct the asymmetry. There will always be subtle differences from side to side but should be able to get things close. I always prepare patients to have more scars but obviously try to do a great surgery with minimal scarring.
