I have underwent silicone microdroplet injections for the trestment of acne scars a few years ago. While the acne scars themselves have been lifted by the silicone, my skin now looks very uneven. Is it safe to undergo fully ablative C02 laser if i have previously had silicone microdroplets injected into my acne scars? I read a paper that said that C02 laser could cause thermal damage around the silicone and i am quite concerned about this posibility?

Silicone, even in microdroplet form, is permanent. Because of this, any heat-based procedure — including fully ablative CO₂ laser — has to be approached with caution. CO₂ laser works by delivering intense thermal energy to resurface the skin, and there is a theoretical risk of heat interacting with areas containing silicone. Some reports and studies have raised concerns about inflammation, damage in the surrounding tissue, or activation of silicone if high levels of thermal energy are applied.

That said, the actual risk can vary depending on several factors:
• how much silicone was used,
• the depth at which it was placed,
• how your skin healed, and
• the specific laser settings being considered.

In many cases, gentler or non-ablative resurfacing options — or even fractional laser settings — may be safer alternatives because they deliver controlled energy without exposing the entire surface to deep thermal injury.
