Well the simple answer is that it depends. If you are far enough out from your surgery and you have been released to full physical activity you absolutely can. I have found in my patients that many have difficulty playing golf with the expanders in as they are firm and can make your golf swing difficult. It is always best to check with your surgeon before trying any physical activity. If you have difficulty playing with expanders, you will likely be fine once they are replaced with permanent implants. Good luck!
If enough time has passed and you have healed well it's ok for you to resume physical activity. The TE are usually placed under the muscle. It may be uncomfortable at first but your body will adjust over time. Please see you plastic surgeon to get more detailed information about your activity level.
Smoking has nothing to do with insurance. You can not have elective plastic surgery for breast reconstruction while smoking because you have a 600% increased risk for a major wound healing problem.
