You need to talk to your neurologist and get approval. I have used Botox and Fillers on patients with MS, but only if their neurologist says its okay. Ask your neurologist and if they say its okay, find an experienced board certified dermatologist or plastic surgeon.
Results last for about 3-4 months, so you can expect the about the same longevity for your results. You can try acupuncture. Just make sure to wait until you've recovered from Botox before getting the acupuncture. 2 weeks is a good timeframe, but you may want to wait 3 just in case.
It is hard to tell from your photo, but it appears that you may get your results by reducing your trapezius and/or platysmal muscles. This can be done with BOTOX. Seek out a board-certified Facial Plastic surgeon for a consultation. Best wishes
