I am a cisgender woman of 27, and I would like to have my nipples surgically removed for a number of reasons: - They cause me physical discomfort, - They make me self-conscious, - I would like to have the areas tattooed, but their sensitivity puts it out of the question. Also, there is no practical benefit for me in having nipples, as I have no intention of having children and I've already undergone sterilization surgery.

Your nipples are not too high, but your breasts have bottomed out from the weight of the implants. You need to go with smaller implants, perhaps an internal bra with mesh for support, and lift your inferior pocket back to its normal position.
If you are concerned because your nipple appears too protuberant to you, it could be reduced in size using the donut excision technique (reduced in size and height).
A breast lift will lift your nipples and areolae so they are no longer pointing down. I would suggest meeting with a board-certified plastic surgeon and discussing your options.
