Accutane not working - what are my other options?

I’m 22, 50 kgs.. my derm put me on accutane after topical tretinoin. Its been more than 6 months being on accutane but i still get episodic break our every one or two week which will last 4-5 days and will get normal again.. I’ve never been on less than 40 mg a day except the first month.. after that I’ve been on 40,80,80,40,40,60, and currently on 80 again..same thing is happening.my derm left it on me to decide if i wanna continue further or 2nd cycle. Idk if accutane didn’t work wht else will
