I weigh 107lbs and I’m 5’3. I wear 30/32A, 13.5 BWD. I feel a little hesitant due to my surgeons suggestion for an 470cc UHP. A different doc told me he could put 475cc high profile; which I liked the sound of, however, this doc will only do a high volume cc if I go with UHP and if I choose HP the most I can do is 385cc. For my frame will 470cc UHP look ridiculous? Will the nipples point in different directions or the boob shape be too projected? Should I just trust my doctors opinion?
Answer: Best breast implants for me? Thank you for the question and pictures. Based on your photographs, I think that you are starting at a good place and should have a very nice outcome with breast augmentation surgery. I also think that your goal photograph is realistic understanding of course that goal photographs are communication tools not guarantees of exact outcome. Online consultants will not be a good resource for you when it comes to selection of appropriate breast implant type/size/profile. Although it is true that ultra high profile breast implants have greater projection, I do not think that anecdotal opinions you have received online are necessarily relevant in your case. In other words, there are many factors that come into play when it comes to the final look achieved with breast augmentation surgery. Anecdotally, I have found that ultra high profile breast implants are very useful when it comes to achieving certain outcomes for patients with narrow frames. ***Given that your surgery is coming up soon, I would suggest that you spend additional time communicating your goals/concerns directly with your plastic surgeon, preferably prior to the date of surgery. Generally speaking, the best online advice I can give to ladies who are considering breast augmentation surgery ( regarding breast implant size/profile selection) is: 1. Concentrate on choosing your plastic surgeon carefully. Concentrate on appropriate training, certification, and the ability of the plastic surgeon to achieve the results you are looking for. ***Ask to see lots of examples of his/her work. 2. Have a full discussion and communication regarding your desired goals with your plastic surgeon. This communication will be critical in determining breast implant size/type/profile will most likely help achieve your goals. In my practice, the use of photographs of “goal” pictures (and breasts that are too big or too small) is very helpful. For example, I have found that the use of words such as “natural” or "C or D cup” or "look ridiculous" etc means different things to different people and therefore prove unhelpful. Also, as you know, cup size varies depending on him who makes the bra; therefore, discussing desired cup size may also be inaccurate. Again, the use of computer imaging has been very helpful during the communication process, in our practice. 3. Once you feel you have communicated your goals clearly, allow your plastic surgeon to use his/her years of experience/judgment to choose the breast implant size/profile that will best meet your goals. Again, in my practice, this decision is usually made during surgery, after the use of temporary intraoperative sizers. Next, we consult with patients and go over the photos and take detailed breast measurements during a physical exam. Next, we using a sizing algorithm to determine a range of implants that will work based on the exam and the measurements.Lastly, during the operation, we use silicone sizers in the operating room to see how the size actually looks in the breast. We sit the patient up in the operating room, while asleep of course, and compare the sizer result with their desired look photos. If it is too small, we start the process over with a slightly larger implant. We don't stop until we get the perfect match.Of all of my breast augmentation patients, every one of them has been happy with their size due to this detailed and meticulous process. Answer: UHP vs HP Thanks for your question and the photos. Based on your wish photos and your current size/shape, I think the Higher profile implant is the better choice. It's hard for me to believe you have a base diam of 13.5cm. I'll be honest, I would prefer the Mentor Moderate Plus Xtra b/c I think it gives a better look. The newest UHP is very projecting and round but that may be what you like. My only caution would be don't go too big and have an implant that is so heavy your breast can't handle it. That is a set up for implant malposition, asymmetry and the need for revision.Best of Luck!

According to your photographs, you seem to want a round, full look, and 470cc UHP will certainly give you that look. My suggestion is to try on sample implants to get an idea of how it will look. In my experience, that is the way to determine the best implant size for you.

Answer: 470cc ultra high profile Hi and thank you for your question. I tend to stay away from ultra high profile implants as they definitely give you more of a torpedo look. Breasts should be round and have a natural shape to them which ultra high profile implants tend not to give. For you I would recommend going with a high profile implant which you could still do a 470cc. That being said, I would caution you a little bit as putting in too large of an implant can be risky. If you try and put too large of an implant in, you risk having your incision open up, having the implant exposed and having to remove the implant for 6 months. If you came to me I would tell you that there's no way I would comfortably put a 470 implant in you and feel like a safe doctor. I would probably steer you towards something in the 300-400 range because that's what I think would be safe. Something else to note though is that the 3D computer programs can be very misleading which is why I don't like using them. Best of luck!
