Tear trough edema: 3 rounds of Hyaluronidase done but nothing changed. What can I do? (Photo)

Had a tear though filler in June 2022 with high density hyaluronic acid. Since then, I have reddish marks under both my eyes at rest, and when smiling or frowning two big lumps under both my eyes. Since January 2024 I’ve gone through 3 rounds of ultrasound guided hyaluronidase per eye, 12 Units every round, to little or no avail: lumps are still quite noticeable in backlights. Ultrasound shows no more nodules, but “there could be more in eye socket”. What else could be done?

Answer: Please understand that ultrasound imaging is a gimmick.
Sometimes temporary gel fillers can lead to scar tissue and nodules that do not readily dissipate. Since this is a temporary gel filler and attempt at dissolving the bumps with the dissolving enzyme may be a reasonable first step. Additionally, dilute triamcinolone acetonide injections may also...
If you were concerned about raised areas where you had injectable filler, if the dissolving enzyme was not helpful, steroid injections maybe useful for decreasing the amount of scar tissue that could be present and causing these raised areas. If that is not helpful, sometimes a shave biopsy or...
Sometimes the appearance of an indentation is due to a bowls from nearby filler. If that is the case, consider asking your doctor for dissolving enzyme to the raised and full area. I hope this helps. Sincerely, Dr. Joseph
