Based on your provided photo your lid margins (where the lashes cross the colored part of your eye) are fairly symmetric and slightly low in both eyes, and thus not surprising that surgeons would mention ptosis repair for you to consider. The supratarsal crease on your right eye is higher than the left, and also has additional wrinkles. It's possible during double eyelid surgery to re-establish folds that are more crisp and symmetric. Your surgeon will be able to show you the effects on your overall appearance from differing crease shapes/heights as well as the additional of ptosis correction.

Hi,Thank you for your detailed question. Based on what you’ve described, your ptosis doesn’t seem to be severe, so it's not absolutely necessary to combine ptosis correction with upper blepharoplasty unless you are aiming for both functional and aesthetic improvement at once.
That said, you can always choose to have ptosis correction later, after your blepharoplasty, if you feel it's needed. It’s also important to understand that achieving perfect symmetry is not possible — since we all naturally have some asymmetry, and surgery can improve but not completely eliminate that.
If your main concern is achieving a more defined crease and youthful upper lid contour without compromising a natural look, upper blepharoplasty alone might be a good starting point. However, this should be carefully evaluated in person, ideally with photos and a physical exam, especially considering your brow dynamics and forehead compensation.
