Old scar keeps on twitching or wriggling?

I have a hypertrophic scar on my lip, whenever i strain my self with too much work or whenever i stop eating, for even about 2 days, i get weaker Whenever i get weaker,the scar twitches, its on my lip and has been there since a long time, is this ever going to change?
