Gaining weight before the procedure is to no advantage and when you lose weight again the fat cells will shrink just like the donor site fat cells. Typically, the transferred fat results in a significant increase in volume of the body site being treated.

Answer: Medical Clearance It's important to be in the best medical condition before undergoing surgery especially for elective surgery. I would recommend seeing your doctor to see if anything could be done to lower your triglycerides prior to surgery. If your doctor feels it is safe, it may be possible to do your surgery. It will depend if you have any medical problems from the high triglycerides and the type of surgery. I recommend seeing a board-certified plastic surgeon for consultation.
