Is there anyway to reduce the width of breasts where they meet the chest wall? I have very wide breasts and have BR questions.

The best technique

but you must make sure your surgeon is aware of your goals and that the plan is altered to accommodate your special request.  Your nipple placement become more challenging as your alter the width of your breasts.
