What is the best option and least scarring (xanthelasma on each corner of eye)? (Photo)

Is it best to cauterize then have an eyelid surgery or will scarring from cauterizing have negative results on surgery Is upper eyelid Ptosis? If excising with surgery will there be a lot of scarring due to position?Please advise how to go about upper lower (bad filler) and Xanthelasma.
