I've been using retin a .05% for two months now and am getting great results on my acne and hyperligmentrion. I've been dermarolling every two months for the past year and have had mild/decent results. What can I do or add that will really smooth out my texture and these old acne scars? Also what kind of acne scars are they? Rolling? Icepick? Would a TCA peel or two be a good option? Would getting a TCA at home peel kit be good?
Answer: Acne scars do great with either of these 2 options. First, there is nothing that compares to a real dermabrasion for acne scarring. 2nd option is Fractora radiofrequency with PRP and Defenage serum. Both will be so worth it.

Answer: Improving acne scars with lasers, microneedling/prp, TCA cross, punch excision, fillers, Cosmelan You appear to have rolling and some ice pick scarring. I don't recommend doing a TCA peel yourself, due to the risk of complications. A combination approach will give the most improvement. These types of scars need a combination of lasers, microneedling/prp, TCA cross, punch excision, Cosmelan and fillers. It will take a series of treatments and may need some treatments to maintain. I recommend getting a formal evaluation with an acne scarring expert. Best, Dr. Emer

Answer: Acne scar treatment Dear NDancer,I would certainly have to examine you but in general, treating acne and the scars takes a comprehensive approach.There are a variety of ways to treat the scars . A good first step is to get on a skin care regiment that normalizes and conditions your skin such as ZO Skin Health. The next step that is relatively inexpensive is a peel or series of peels.The next step up would be laser resurfacing such as fraxel. Even with laser, some of the "ice-pick" scars still subsist and remain. I've found in my practice that local subscision, using a needle to elevate the scar followed by fat grafting gives the best results for these deeper acne scars Large pores and acne scarring can be treated effectively with combination therapy. I would suggest coming in for a consultation to see in person but generally speaking we've had great success with a combination of microneedling, light fillers, and laser therapy. Answer: Acne scarring Your scars actually aren't that bad. We've successfully treated much worse cases. Depending on how deep they are, dermal fillers may be required at first. In your case, likely not. After that, a simple microdermabrasion, followed by micro-needling (with or without PRP) will give you excellent results. Be aware that a series of treatments provides the best results. As always, make sure you consult with an experienced practice so you can get the best list of options.

If they are still young and remodeling, you can continue treating your scars with various topical options such as silicone-based strips or gels and most importantly - keep them out of the sun. Overtime, your scar should continue to improve on its own. For young,old, or deeper scars that have plateaued in their improvement, microneedling with or without PRP, fractionated laser, and radiofrequency make microscopic holes in the scar to stimulate your body to remodel and fill in the scar with new collagen. I especially like a combination of a chemical peel for the hyperpigmentation and acne control, microneedling with PRP for texture and collagen building, and PIXEL fractionated laser or RF to resurface and build even more collagen. Not everyone is a candidate for all the treatment options however as certain skin types can get worse from certain treatments.
