Dear Julielyn93; It's fairly soon after your August surgery, to consider some type of revision surgery. But at this point, you should be thinking about what the options are in anticipation of the proper time. It may well be that in another month or two, the nostril asymmetry may get better, so I would advise you to “hold tight.” Do not do anything yet.I t is amazing how sometimes things can loosen up a bit, settle down and improve. I am sure your surgeon feels the same way. Typically, surgical revision rhinoplasty is not considered for at least six to nine months after surgery. However, depending on the particulars of what is happening with the nostril area, it may be appropriate to perform a non-surgical or filler revision rhinoplasty earlier. It all depends on how the tissues look and feel, and all this calls for a very careful examination by your surgeon or a consultant, if that's the course you want to pursue. With respect to one side being more “depressed” than the other, that too may self-correct in time, and I would advise waiting. Often the tissues are too firm to have any intervention performed upon them, so try to be patient if you can. As far as the fillers go, it is certainly a consideration and you need to consult with somebody who is very experienced and can offer you that option. Of course, first check with your surgeon, because he knows your nose best.Silikon 1000 (Adatosil) is an inert, medical-grade, liquid silicone cleared by the FDA for use inside the eye. Cosmetic surgeons may legally use it under “off label” provisions that say, in the doctor’s best judgment, a medical device or product may be used if it uniquely fills a patient’s needs. In the right hands, micro-droplets of Silikon 1000 are meticulously placed in just the right places under the skin. If a small bump (medically, a nodule) develops at the injection site, the droplet has been placed too shallowly under the skin. Critics claim liquid silicone travels under the skin, but that happens only when it is injected in large pools. One Silikon 1000 micro-droplet injection deposits 1/8th the amount an eyedropper dispenses in a single drop.Finding a good cosmetic plastic surgeon starts with a computer search of surgeons in your (or distant) areas. Best results come from super-specialists (a medical, not an advertising term) who do the procedure you want at least weekly and have done it for at least a decade. Once you narrow in on the website of a surgeon who looks good to you, focus on the before and after pictures, looking for patients who have had the procedure you want. There should be dozens and dozens of such photos. Many good, plain language books about cosmetic plastic surgery are on Amazon.com or Barnes&Noble.com. As you read, write down questions that can be asked later in consultation with a surgeon. So do that homework. You live in a large metropolitan area where you should be able to find surgeons who operate that super-specialist practice focusing on noses and who have the facility to do what you want to have. Best wishes, Robert Kotler, MD, FACS
